Those slings try to imitate what peasant women carried their children in. Strap the kid to your back and go to work in the fields. They are good for very young babies because the baby feels close to the mother's heartbeat, as it was in her body. After they get a little older, perhaps they will use some other device for carrying, even hands and arms.

River also looks like he is a bigger baby and keri is a small framed woman so this is also helpful on her back and shoulders. Hence the sling, is better for Mommy to be able to carry him safely & still be close to him. Keri is a VERY small framed woman, River weighs AT LEAST 18 lbs!! Its NOT easy to carry around 18 lbs all the time like that. It weighs on the back, shoulder, etc. sheesh!!
Maybe it's a tribute to Keri's mother more than Keri. And maybe the fact that Keri has had a good career but has not been the number one pop star in the world lends to some stability and normality. Not talking against her, just saying she has not dealt with the amount of fame and adoration that Britney has at a very young age.
There was a trend in Hollywood for awhile that women would not have babies but would only adopt, to preserve their figures, it seemed. This trend seems to have reversed itself, with motherhood being right up there with pursued prizes.
